Fatal Accident Claims

Important Information.
Sections 1, 2, 3, & 5 of the claim form should be completed by you and Section 4 should be completed by either your GP or the medical consultant. We do not pay any fees charged by your GP or consultant for their completion of the form.
If we need to obtain further medical evidence we’ll always let you know who we have written to and will keep you regularly updated on the progress of your claim. The cost of any additional reports we require will be paid by us.
Please provide a copy of either the interim or final death certificate with the completed report form.

Confirmation of the incident - official reports or third party correspondence
